Talk with us CARE PROGRAM GUIDE · APCM
Advanced Primary Care Management
A patient’s primary-care team keeps the whole care plan connected over time, including questions, medicines, referrals, and changes after hospital visits.
FIRST, THE SHORT ANSWER
What would APCM add to care?
APCM is a monthly primary-care payment bundle for practices that serve as the continuing focal point for the patient’s overall care and furnish a broad set of access, planning, coordination, communication, and population-management capabilities.
APCM requires electronic care planning, information exchange, enhanced communication options, and population-level analysis; it does not require a monitoring device or minute-by-minute time ledger.
PATIENT & PRACTICE FIT
Could this help people you treat?
Potential fit
An eligible primary-care practitioner and practice able to provide the full APCM model, not just one monthly contact; the patient has given informed consent.
Check before enrolling
The practice is not the continuing focal point for all needed care; required access or population-management capabilities are absent; or the proposed claim merely relabels another monthly service.
The patient can reach the practice for urgent needs, has an electronic comprehensive care plan, and sees care transitions and specialty/community services coordinated through a consistent team.
SEE IT IN PRACTICE
Care between visits, step by step.
Meet Grace: her primary-care practice is her main point of contact for several health needs and specialist appointments.
The goal: Give Grace one coordinated primary-care plan and a reliable way to get help.
Choose a stage to see what the patient, treating practice, and PrimeVital’s agreed team each do.
Check readiness
Before enrollment- Patient / caregiver
- Discusses consent, choice of provider, costs, and stopping services.
- Treating practice
- Confirms the primary-care relationship, initiating-visit status, and full model capabilities.
- PrimeVital’s agreed role
- Helps assess gaps and scope the care-management arrangement.
- What gets recorded
- Primary-care ownership, consent, tier, access arrangements, and reporting pathway.
Illustrative scenario, not a patient record or clinical protocol. Clinical targets, review frequency, staff qualifications, escalation, and service hours are agreed with the treating practice. Clinical decisions stay with qualified personnel.
SAMPLE CARE PLAN
A plan the patient and the team can understand.
Give Grace one coordinated primary-care plan and a reliable way to get help.
- Patient task
- Use the agreed contact routes and share health changes, priorities, and barriers.
- Clinical owner
- Her primary-care practitioner remains the focal point for all needed care.
- Team follow-up
- Coordinate medicines, prevention, referrals, transitions, and the shared electronic plan.
- Review
- Update her plan as needed and review population gaps and performance through the practice’s reporting process.
After launch: the continuing period
Keep capabilities available and deliver elements when clinically appropriate. APCM is not just CCM with its time ledger removed. The practice’s 24/7 access requirement is not a promise of 24/7 PrimeVital staffing.
YOUR INVOLVEMENT
Clinical ownership stays with the treating practice.
The agreement names the people, work, coverage hours, records access, and handoffs. Technology and operational support do not transfer clinical or billing authority.
What your clinician does
An eligible physician or other qualified primary-care practitioner directs the service and remains responsible for all primary care and the APCM claim.
What supporting staff can do
Clinical and auxiliary personnel may furnish applicable elements under the billing practitioner’s direction and current supervision rules. The practitioner and practice must actually maintain the APCM capabilities.
What we can organize with you
PrimeVital can help assess readiness, organize care-plan and transition workflows, patient outreach, staff routing, and evidence collection. It cannot replace the practice’s primary-care relationship or leadership.
PrimeVital’s full care arrangement includes applicable devices and a care-management team. The program determines which resources and professional qualifications are needed. Interactly can support front-desk tasks within the agreed scope; 24/7 PrimeVital clinical coverage is not assumed.
REQUIREMENTS BEFORE LAUNCH
Make the pathway ready before enrolling patients.
- Confirm the billing practice is the continuing focal point for all needed primary care.
- Obtain consent. For a new patient, complete an initiating visit unless the same practice saw the patient within three years or provided APCM, CCM, or PCM within the past year.
- Maintain 24/7 access, electronic care plans, transitions, enhanced communication, population analysis, and performance reporting.
- Select the correct patient tier and screen for services included in the APCM bundle.
Detailed operating workflow and technology
- 1
Confirm
Check the primary-care relationship, available capabilities, patient tier, consent, and initiating-visit status.
- 2
Organize
Maintain comprehensive electronic plans, access coverage, transition workflows, and enhanced communication.
- 3
Manage
Use population data to find gaps and coordinate individual patient needs.
- 4
Measure
Document applicable monthly elements, reporting obligations, and the correct patient-complexity tier.
Technology and records: APCM requires electronic care planning, information exchange, enhanced communication options, and population-level analysis; it does not require a monitoring device or minute-by-minute time ledger.
GETTING PAID
Who receives payment, and what earns it?
Care need, completed services, evidence, and the actual payer determine the claim. A covered service or an enrollment is not the same as collected revenue.
The qualifying primary-care practitioner reports the appropriate monthly APCM bundle. Embedded CCM, PCM, TCM, and communication work is not an automatic extra claim. PrimeVital’s service cost is separate from payer collections; QMB protections must be respected.
Original Medicare Part B pays APCM as a monthly bundle. It is not time based. CMS requires consent and an initiating visit for new patients unless specified recent-care exceptions apply.
Medicare Advantage, Medicaid, ACO, and commercial value-based agreements may already pay for related population management. Confirm attribution, contract, and duplicate-payment rules.
CMS requires consent explaining that cost sharing may apply. QMB protections are relevant to G0558; do not charge a QMB patient prohibited Medicare cost sharing.
Payment boundaryAPCM incorporates elements of CCM, PCM, TCM, and selected communication services. Do not separately bill embedded work as though it were independent; review the current CMS add-on and concurrency instructions.
2026 code reference and an illustrative service check
Choose the supported patient tier and check the full practice model. There is no monthly minute threshold to reach. A higher condition count does not make embedded CCM, PCM, or TCM work separately payable.
These are code-family summaries, not complete descriptors or payment estimates. Check the current code set, payer, clinician, setting, date, unit edits, and required add-on base code.
| Code / pathway | What it describes | Before billing |
|---|---|---|
| G0556 | Base APCM level | Primary-care model for a patient with zero or one chronic condition. |
| G0557 | APCM for two or more qualifying conditions | Conditions meet CMS duration and risk criteria. |
| G0558 | APCM for qualifying QMB patient | Two or more qualifying conditions plus Qualified Medicare Beneficiary status. |
| G0568 / G0569 / G0570 | Optional BHI/CoCM add-ons | Only when all additional service elements are met and the same practitioner reports an APCM base code in that month. |
Questions before a claim
Is APCM just CCM without time tracking?
No. It is a broader primary-care model with specific access, coordination, population-management, and reporting requirements.
Does the practice have to perform every element every month?
CMS says elements are furnished when clinically appropriate; the practice must have the model capabilities in place.
FINANCIAL PICTURE
See how participation, collections, and costs change the result.
Start with one supported pathway. Do not multiply every patient by every possible code. Use verified payer terms and the written PrimeVital scope before making a business decision.
These starting figures are hypothetical. They are not Medicare rates, a PrimeVital quote, or a payment forecast. PrimeVital pilot terms are not modelled automatically. Replace them with your verified contract, service mix, and cost assumptions.
Estimate net after your agreed service fee and other costs
35 of 50 patients meet the assumed service requirements.
Illustrative collections before service and practice costs: $3,500. This uses the hypothetical per-patient amount and collection rate above; it is not a code-specific Medicare estimate.
To estimate the amount remaining for the practice, open the cost section and enter the total service fee from a written proposal. We do not invent a PrimeVital price or show a misleading net amount before that is known.
Contribution is before other overhead and taxes. Actual billing uses service-level evidence and rules; this simplified model does not select codes, approve eligibility, or guarantee outcomes. Patient cost sharing and QMB protections must be checked separately.
BENEFITS TO MEASURE
Define success before the first patient starts.
Agree on a baseline and review period. These are measures to track, not promised results or guaranteed payment.
Access
Use of agreed contact routes and continuity with the care team.
Care gaps
Preventive services, referrals, and post-discharge follow-through.
Practice performance
Required quality and cost reporting alongside patient experience.
